Updated consent forms separating different kinds of consent, are now available.

• Consent for a Third Party to Act on Behalf of a Participant form

Participants and prospective participants can complete one or both forms, depending on type of consent they want to give to NDIA

The forms replace the NDIA for Consent to Exchange Information and Consent to Share Plan with Provider form
